Physician Resources and Training

Building Your Medical Home Toolkit
The Toolkit supports your development and/or improvement of a pediatric Medical Home. It also prepares you to apply for and potentially meet the National Committee for Quality Assurance (NCQA) Physician Practice Connections’ Patient Centered Medical Home (PPC-PCMHTM) Recognition program requirements. The AAP created a crosswalk between each of the Toolkit building blocks and the NCQA PPC-PCMH Recognition Program ‘must pass’ elements.

NCQA’s Patient-Centered Medical Home
The NCQA now recognizes clinician practices functioning as medical homes by using systematic, patient-centered and coordinated care management processes.
NCQA’s Patient-Centered Medical Home (PCMH) is an innovative program for improving primary care.

Center for Medical Home Improvement
Center for Medical Home Improvement is dedicated to helping community-based, primary care practices and networks to expand their ability to offer community-based primary care setting which provides and coordinates high quality, planned, family-centered health promotion, acute illness care, and chronic condition management – the standard of excellence for pediatric and adult primary care in the 21st Century.
