The stories shared here show how that alignment shapes daily life.

Families often reach out when care feels disconnected. Appointments are spread across providers, schools raise concerns, and information does not move between systems. Parents and caregivers are left trying to connect the pieces.
When communication improves and plans are aligned, that experience changes.
Thanks to our coordinated System of Care, our partners can more effectively support families when a child’s needs involve more than one system at the same time (such as healthcare, school, community services, or justice partners).

Baby Troy was diagnosed with a small lower jaw and a cleft palate, which initially created breathing problems.
Following the ACPA guidelines the team was able to see him within the first week of life to develop a treatment plan. “Initial team follow up is critical for the newborn’s optimal development, as well as aiding the parents understanding of the newborns needs in relation to his cleft diagnosis,” said Lida Sarnecky, Program Nurse Manager.
“Prior to surgery, Baby Troy had tremendous sleep apnea and now his Mom says he sleeps as quiet as a mouse.”

The work of connecting healthcare providers, schools, courts, and community organizations often happens behind the scenes, yet it shapes what families experience every day.
When partners communicate and follow a shared plan, care moves forward with fewer gaps and clearer next steps. Over time, that consistency makes a measurable difference.
If you are seeking support for a child or family, this is a place to begin.
