These resources are provided by the Center for Collaborative Care (CCC), a partnership between pediatrics and psychiatry that supports children, families, and care providers across Northeast Florida. More than 100 pediatricians, nurse practitioners, and physician assistants from 20+ pediatric and family medicine practices in Northeast Florida are involved in the CCC. Through collaboration with primary care partners, the Center helps connect communities with trusted information and tools that support informed care.
